You pulled out every trick you knew.


Cut the carbs. Increased the workouts. Watched every calorie.

And the weight kept coming.


Dr. Janeeka Benoit knows this feeling.


She gained 20 pounds in her mid-30s. Gradually, quietly, until the pants she had always worn would no longer come above her waist.


She did a double take. Checked the label. Same pants.


She had been a personal trainer before medical school. She is a sports medicine physician. She knew exactly what to do.


And none of it was working.


So she went harder.


Restricted her calories more. Exercised more. Pushed herself to the extreme.


Because that is what high achievers do when something stops working.


We do not ease up. We go harder.


She said it herself: that is also when we can do ourselves the most damage.

Here is what the research is now saying clearly.


When estrogen drops during perimenopause and menopause, muscle mass drops with it.


Both the frequency and intensity of muscle loss increase during menopause, and this progression has a name: sarcopenia.


Estrogen plays a role that extends far beyond reproduction. It influences skeletal muscle, mitochondrial function, and the body's ability to maintain muscle mass and strength.


The decline in estrogen after menopause significantly contributes to increased visceral fat and reduced muscle strength.


That visceral fat is not just about appearance. It is the inflammatory type of fat that raises your risk for metabolic syndrome.


So when you are doing everything right and your body is not responding the way it used to, this may be part of what is happening.


Your hormones shifted. Your body composition shifted with them.


That is not a discipline problem. It is a biology problem.




This Week on the Podcast


I sat down with Dr. Janeeka Benoit this week.


Board-certified internist and sports medicine physician. Telehealth practice owner. And someone who went through this herself before she helped her patients through it.


We talked about what happens when a high achiever's body stops responding to the things that always worked.


We talked about muscle. Not the kind you build for aesthetics. The kind that carries you through the next 40 years.


Dr. Benoit made a distinction I want you to hear.


There is a difference between training for how your body looks and training for how your body functions.


Squatting to pick up your groceries. Getting up from a chair without holding the armrests. Lifting something heavy off the floor without throwing out your back.


That is the training that matters most right now. And it looks different from what most of us have been told to do.


She also talked about a patient who came to her already on hormone replacement therapy, still not sleeping, still not recovering from workouts.


Dr. Benoit told her to cut back from four workouts a week to three.


The patient looked at her like she was out of her mind.


But the soreness improved. The sleep improved. And she started making progress.

Because you cannot build muscle on a body that has not recovered.


She also shared something that stopped me.


Sometimes the sleep issue is not just hormonal.


For this patient, something that happened 15 years ago was keeping her nervous system on guard at night. Unable to rest. Unable to recover.


Hormone therapy alone was not enough. It took a multimodal approach to get her there.

That story alone is worth the listen.
