Nobody brings this one up in the exam room.
Not because it is not real.
Not because it is not affecting their relationships or their sense of self.
But because the shame around it makes it easier to just not say it out loud.
I have watched women list every symptom they are experiencing.
Brain fog. Sleep disruption. Mood swings. Weight gain.
And then on the way out the door, almost as an afterthought, they pause and say.
Also. I just. I don’t really want to be intimate anymore.
Is that normal?
And I stop them right there.
Because that is not a footnote.
That is not a bonus symptom.

Up to 68% of postmenopausal women report a decrease in sexual desire.

What’s in the News
The missing piece of the libido conversation is testosterone.
And the research is making a strong case for why that needs to change.
A 2024 study published in the Archives of Women’s Mental Health looked at 510 perimenopausal and postmenopausal women already on HRT but still experiencing low libido, low mood, and brain fog.
All were treated with transdermal testosterone for four months.
More than half reported an improvement in libido.
Nearly half reported improved mood.
Over a third reported clearer cognitive function.
Testosterone declines right alongside estrogen and progesterone during the menopausal transition.
It directly affects desire, arousal, energy, and brain clarity.
And most providers never bring it up.
